Do I qualify?

You qualify for CCA One Care if all of these apply to you:

  • You’re between 21 and 64 years old. 65 or older?
    See if CCA Senior Care Options (HMO D-SNP) is right for you.
  • You have MassHealth Standard or MassHealth CommonHealth.
  • You have Medicare Parts A and B.
  • You qualify for Medicare Part D.
  • You don’t have private health insurance.
  • You live in one of these counties: Barnstable, Berkshire, Bristol, Essex, Franklin, Hampden, Hampshire, Middlesex, Norfolk, Plymouth, Suffolk, or Worcester.
